FAQs on the urology family
Do PROMs have to be recorded in the nephrectomy register?
No, PROMs are not available in the nephrectomy registry.
Is there a deadline for data entry?
There is no deadline for the SwissUro project itself. However, there is one for the SwissUro audits, namely March 1 of the following year. This date also applies to the Zurich Health Directorate’s urology project. It is likely that the same date will apply to the recording of HSM-relevant operations.
How should a case be documented if an additional prostate cancer was detected during a cystectomy?
Additional prostate cancer can be documented postoperatively in the “Urology Oncological Cystectomy” questionnaire. If the field “additional prostate cancer” is answered with yes, the corresponding Gleason scores and ISUP grades as well as the pTNM information appear. It is not necessary or possible to switch to the prostatectomy questionnaire “Urology Prostatectomy”.
Should patients with nephroureterectomies be included in the questionnaire?
No, these patients cannot and should not be included in the questionnaire, as the necessary fields or codes do not exist.
Should nephrectomy patients with a non-functioning kidney be included?
No, these patients should not be included in the registry.

PROMs cystectomy register:
For the cystectomy registry, there are several PROMs for preoperative (approx. 2 weeks before the operation) and postoperative to choose from, which are to be completed by the patient 3, 12 and 24 months after the operation, and optionally annually thereafter. Update 08.07.2021: The order of the questions in the cystectomy PROMs has been changed. From now on, the SF12 questions will be asked at the beginning and the FSFI or SHIM questions at the end.
PROM as part of the “Prostate Cancer Quality Assurance of the Zurich Health Directorate from 2020”:
From January 2020, the Health Directorate of the Canton of Zurich (GD ZH) will require quality assurance in the treatment of prostate cancer. Patients who have undergone a prostatectomy are provided with a PROM preoperatively (approx. 2 weeks before the operation) and postoperatively, which must be completed 3, 12 and 24 months after the operation, and optionally annually thereafter.
The questions to be answered by the patient are part of the EPIC-26, which is why Zurich hospitals can also choose to have the official PROM of the SGU completed.

Änderungen per 18.03.2022:
Im Fragebogen Prostatektomie 2022 wurde bei der Eingabe des postoperativen T-Stadiums (Feld 6761) zusätzlich der Code T2 aufgeschaltet.
Änderungen per 10.02.2021:
Im Fragebogen Nephrektomie 2019–2021 gab es die folgenden Änderungen hinsichtlich der Unterscheidung von malignen und nicht malignen Tumoren
- The following codes have been deleted from field 4798 “Pathological diagnosis”:
- 6 “Metastases”
- 7 “Angiomyolipoma”
- 8 “Oncocytoma”
- 9 “Squamous cell carcinoma”
- The new code 11 “no malignant renal tumor” was added to field 4798.
- The field 4797 “Longest tumor diameter (mm)” was renamed “Longest pathological tumor diameter (mm)”.
- The field 4986 “measured with” (in the sense of “how was the tumor diameter measured?”) has been hidden
- If there is no malignant tumor, i.e. the new code 11 “No malignant renal tumor” was selected in field 4798 “Pathological diagnosis”, then field 6493 “Other pathology” appears. If the code 8 “other” is selected, the text field 6494 appears. Furthermore, after saving, each follow-up is hidden or answered with “no”.
Changes as of 22.01.2021:
Concerns the GD Zurich questionnaire and the combined questionnaire 2021:
- The new field 8973 “Tumor board held” has been added. If this field is filled in with “no”, the following fields are hidden and do not need to be filled in:
- Field 5520 “Tumor board recommendation pre-therapeutic”
- Field 5569 “Information and consent to treatment”
- Field 5570 “Different tumor board decision before therapy available”
- Box 5529 “Date of therapy discussion (patient information) and transmission of tumor board radiation – therapy decision: Written report by the physician providing information on all alternatives and decision”
- Field 5603 “Therapy chosen by the patient after information”
- Field 8973 is mandatory.
- In 2020, the same change was made to the same questionnaire, but instead of field 8973 there is field 8554 “Tumor board recommendation pre-therapeutic”.
Changes as of 21.01.2021:
Concerns the DG Zurich questionnaire 2020 and 2021:
- The new mandatory field 8974 “Biopsy performed” now shows the following fields by entering “yes”:
- Field 5505 “Multiparametric MRI before biopsy”
- Field 5507 “Type of biopsy”
- Field 4350 “Date of biopsy”
- Field 4350 “Number of punching cylinders removed”
- Field 4353 “Gleason score value 1”
- Field 4354 “Gleason score value 2”
- The above fields do not have to be filled in and remain hidden if the mandatory field “Biopsy performed” was filled in with “no”.
Changes as of 07.01.2021:
Concerns all cystectomy questionnaires:
- The field 4779 “additional C sharp” now always appears, regardless of gender.
- The field 4785 “additional C sharp” now always appears, regardless of gender.
- The field 4784 “Extracapsular extension of positive lymph nodes” now appears if the code N1, N2, N3, or NX was selected in the field 4786 “N stage” (it does not appear for N0).
- The codes T1, T2a and T2b are now also available for selection in field 4413 “T stage (preoperative)”.
- The code T2 is now also available for selection in field 4875 “T stage (postoperative)”.
- A help button has been added to field 9122 “Length of stay in intensive care unit (days)” with the following text: “If a patient was immediately readmitted to the intensive care unit due to complications after the postoperative stay, both stays must be added together.”
- A help button has been added to field 4993 “Nerve sparing performed with the text: Partial nerve sparing is considered nerve sparing.”
Concerns all prostatectomy questionnaires and GD Zurich questionnaires:
- If field 5507 “Type of biopsy” has been filled in with code 5 “TURP”, then field 4350 “Number of punch cylinders removed” is no longer displayed and no longer needs to be filled in.
- The field 4920 “PSA preoperative (ng/ml)” was extended by one digit before the decimal point (NU4).
Unless otherwise stated, the following changes affect both the questionnaire “Urology additional filter GD Zurich” and “Urology combifilter SGU / GDZH”:
- The code T0 was added to field 4875 “T stage” (postoperative/posttherapeutic). The change also affects the 2020 questionnaires.
- Field 5520 “Tumor board recommendation pre-therapeutic” was changed from single-choice to multiple-choice. The change only affects the year 2021.
- Field 5514 “Assessment of life expectancy (1 – 100 years)” has been renamed “Estimation of life expectancy (1 – 100 years)”. The change also affects the 2020 questionnaires.
- The code 8 “Systemic therapy” has been added to field 5603 “Patient’s chosen therapy after information”. The change also affects the 2020 questionnaires.
- Code 5 “TURP” has been added to field 5507 “Type of biopsy”. The change also affects the 2020 questionnaires.
- The mandatory field 5512 “Doctor who determines the tumor stage (surgeon number)” has been converted into an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5559 “Optional participants tumor conference / pathology / specialist involved?” has been changed to an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5560 “Optional participants tumor conference / nuclear medicine / specialist involved?” has been changed to an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5517 “Determination of patient’s operability after tumor board by primary care provider” has been converted into an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5518 “Determination of patient’s operability after tumor board by specialist in general internal medicine” has been converted into an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5519 “Determination of patient’s operability after tumor board by anaesthesiology specialist” has been converted into an optional field. The change also affects the 2020 questionnaires.
- The mandatory field 5516 “Date of pre-therapeutic tumor board with treatment decision/recommendation” has been converted into an optional field. The change also affects the 2020 questionnaires.
- Mandatory field 119 “Teaching assistance” has been converted into an optional field. The change only affects the questionnaire “Urology additional filter GD Zurich” and also applies retroactively for the 2020 questionnaire.
- The conditional mandatory field (depending on field 119 “Teaching assistant”) 118 “1st assist. Surgeon or 2nd surgeon” has been converted into an optional field. The change only affects the questionnaire “Urology additional filter GD Zurich” and also applies retroactively for the 2020 questionnaire.

03.12.2019 Adjustments to the questionnaire
The field [7588 4986] “measured with” has been expanded to include code 4 “pathology”.
26.09.2019 Adjustments to the questionnaire

24.09.2019 Adjustments to the questionnaire
- The field [62] “Histologic grading” (preoperative) has been given the new field number 7999. The codes have been changed to 1 “High Grade” and 2 “Low Grade”.
- The codes of the field [4786] “Histologic grading” (postoperative) were changed to 1 “High grade” and 2 “Low grade”.
- The field [4770] “Specify therapy” was expanded to include code 3 “Cisplatin-based and immunotherapy”.
- Code 3 “BCG and chemotherapy” was added to field [4772] “Type of instillation”.
- New check: If the field [4876] “N stage” was filled in with the code N1 or N2, then the field [4363] “Number of affected lymph nodes” must not be filled in with 0
24.09.2019 EPIC-26 questionnaire
The former questionnaire in the prostatectomy registry (inserts + SHIM + SF12 + ICS) was replaced by the EPIC-26 questionnaire (Expanded Prostate Cancer Index Composite).
In addition, the fonts in all PROMs have been enlarged and the information

- (planned) entry date
extended.
13.08.2019 CKD-EPI formula
The fields “GFR according to Cockcroft-Gault (ml/min/1.73m2)” in the nephrectomy filter have been changed to “GFR according to CKD-EPI-creatinine formula (ml/min/1.73m2)”. The calculation is performed according to the scheme at http://ckdepi.org/equations/estimating-equations/creatinine-based-equations/. For this purpose, the new field [7736 5230] “Skin color” with the codes 1 “black” and 2 “not black” was added.
The field number 5060 has been changed to 7737. All other fields for calculating the GFR retain the same field number.
