Structure of our forms
The Adjumed system has a total of two dimensions, one of which has three levels, with which every hospitalization of a patient can be fully mapped. It is also possible to store master data on departments, patients, doctors and external clinics.
Here we provide an illustration of the individual levels using the AQC family as an example:
1st dimension, 1st level A0:
e.g. hospital section: At level A0, all admission and discharge information including diagnoses of a patient during hospitalization are normally recorded.
1st dimension, 2nd level A1:
e.g. surgery section: At level A1, for example, all surgeries performed on a patient during hospitalization are recorded. This level can include 0 – n operations.
1st dimension, 3rd level A2:
e.g. intervention part: At level A2, for example, all intervention parts during an operation are recorded. An operation can consist of several procedures or parts of procedures. This is the case, for example, if a change of surgeon has taken place during an operation or if a complication occurs during an operation that is to be assigned to a specific part of the operation.
2nd dimension, 2nd level B1:
e.g. children: The case with the operations (e.g. caesarean section and additional operation on the following day) is dealt with in the first dimension, the twins in the second dimension.
Each case in the Adjumed system represents the complete hospitalization of a patient. This means that there is always exactly one case per patient and hospitalization. The patient is uniquely identified in the system per clinic/center by the patient number (PID) and the date of admission/start of case. Several cases can be assigned to a patient with the help of the PID.
An entry date and a PID are mandatory for the identification of a specific case. (Exception: only a case identification number (CID) is sufficient for the web service import).
As the customer, you determine which dimensions and levels your registry consists of and which fields and codes you want to use with your recording and evaluation requirements.
We will be happy to advise you on setting up or changing your registry. In this dialog process, you as the customer bring in your requirements and we bring in our experience as to which fields and codes should be collected when and where.
