Adjustments to the forms and the transfer files are usually made once a year. If a year is missing from the listing, no adjustments have been made during that period.

Please note: A case starts with the date of the first visit and lasts for a maximum of 365 days or until the change of the main complaint.

Only the questionnaire at the first visit should be used for the initial survey at case start. From then on, the survey runs automatically. The follow-up questionnaire to be filled in after case end or in case of main reason change will be sent automatically.

The letters for the FollowUp survey need to be sent through the center. Because the letters have the Adjumed Services sender on the letterhead due to the patient’s address, a hospital cover sheet should be included. Such a sample accompanying sheet from Inselspital can be found in the Appendix.

The survey can now also be started automatically from AdjumedCollect. I.e. one changes to the filter “Endo PROM Initialization” and clicks that the patient should be interviewed. Then you save and get back both a direct link and a QR code scannable by, for example, a cell phone or tablet.
